      <image:caption>“Blef-a-ro-plasty” is a surgical procedure aimed to rejuvenate the appearance and improve the function of the eyelids. As we age, the skin of the eyelids becomes thinner and stretches resulting in excess skin (dermatochalasis). Fatty tissue around the eyes can become more prominent and more apparent through the thin tissue. These aging changes make us look tired and older than our actual age. Blepharoplasty is a simple, outpatient procedure that can restore the eyelids to a more rested and youthful appearance. Occasionally the eyelid skin can become so severe that it may block the upper field of vision, in which case the surgery can eliminate the obstruction and improve the peripheral field of vision.       <image:caption>Dr. Zachary Joos is a board certified ophthalmologist and fellowship trained oculofacial plastic surgeon specializing in eyelid and facial plastic surgery. Originally from Bellevue, Dr. Joos completed his undergraduate studies at the University of Southern California on an academic Dean’s scholarship. He obtained his medical degree from Creighton University School of Medicine in Omaha, Nebraska graduating with honors. He completed an internship in Internal Medicine at the University of Utah Hospitals and followed by an Ophthalmology Residency at the John A. Moran Eye Center at the University of Utah in Salt Lake City. Dr. Joos completed two fellowships, the first in advanced anterior segment surgery (complex cataracts, ocular trauma) at the Moran Eye Center where he also served as the Associate Residency Program Director while teaching surgery to residents at the Salt Lake City Veterans Administration. He then completed a fellowship in Oculoplastic, Orbital and Lacrimal surgery at The Royal Brisbane &amp; Women’s Hospital in Brisbane, Australia. (Preceptor: Professor Timothy J. Sullivan, FRANZCO, ASOPRS) Dr. Joos enjoys spending time skiing, hiking, travel, soccer and spending time with his wife and three children. Approximately 50% of Asians have an eyelid crease giving the appearance of a “double eyelid.” Although there is a crease present in these 50% of patients, the eyelid crease is much lower than in “western” eyelids. It is critical that the eyelid crease remain low when performing Asian blepharoplasty to avoid an unwanted “westernized” appearance. The Asian eyelid can have a crease of various shapes and up to 50% have no visible crease.  It is important to talk to your surgeon about the height and type of eyelid crease you desire before undergoing surgery. The goal of surgery is to create a natural crease and eyelid that fits with the unique face of the patient. This procedure reduces the redundant skin and returns the earlobe to a more normal shape, size and contour. This procedure is usually performed in an office setting under local anesthesia. The procedure takes only 30-60 minutes to complete both sides. The earlobe will be repaired using a thin line of sutures after which the area heals with a thin linear scar that fades over time. After care for the incisions consists of using antibiotic ointment daily for 1 week and keeping the area clean. Ice is recommended to the area for 48 hours to reduce swelling. Normal activity can resume after about a week. The ears can be re-pierced after 2-3 months if desired.       <image:caption>Dark circles under the eyes are caused by several factors. The eyelid skin is the thinnest on the body which makes underlying structures-such as veins- visible just beneath the skin. Loss of volume and descent of the cheek fat pads can also cause a depression, known as the “tear trough” which creates a clear dividing line between the eyelid and the cheek. Frequently, herniation of fat about the tear trough causes shadowing just below the fatty bulge resulting in a dark circle. This is most noticeable in overhead lighting, which is common in most social situations. As you can see, there are several reasons these dark circles occur. Because of this, it is important to see a specialist with training in eyelids and the delicate tissues around the eye, such as an Oculoplastic surgeon. When injecting this area with filler this area, it is critical to select the correct type of product. Because of the thin skin and underlying tissue, it is best to avoid products that draw in a lot of water (hydrophilic) such as Juvéderm.  I prefer products such as Restylane and Belotero and fill these areas conservatively with a blunt-tipped cannula. In addition to proper product selection, the product must be placed in the correct location. Your surgeon can advise you as to what the best treatment is, such as dermal filler versus more invasive treatments such as surgery.       <image:caption>Ptosis (pronounced “toe-sis”), or blepharoptosis, is the medical term for drooping of the upper eyelid. This reduction in the size of the eyelid opening may cause reduced field of vision because the eyelid crosses part, or all, of the pupil. Patients with ptosis often have difficulty opening their eyelids completely and frequently lift their eyebrows to compensate for this deficit. Ptosis is frequently asymmetric between the two eyes. As we age, the skin of the eyelids becomes thinner and stretches resulting in excess skin (dermatochalasis). Fatty tissue around the eyes can become more prominent and more apparent through the thin tissue.These aging changes make us look tired and older than our actual age. Blepharoplasty is a simple, outpatient procedure that can restore the eyelids to a more rested and youthful appearance. Occasionally the eyelid skin can become so severe that it may block the upper field of vision, in which case the surgery can eliminate the obstruction and improve the peripheral field of vision.
